The form requires applicants to fill up 24 fields consisting of name, gender, age, birthplace, residential address, occupation, date of joining the KMT, where the member joined, the proposer, the Party's registration number and the District and Branch to which the member belonged; also includes a photo of the member, a series of questions for applicants to answer and "office use only" details.
